There are two types of offence notices (tickets).  To identify which one you have received:

Sample 1

If your ticket is 8 1/2 X 11, complete the right side of the ticket and scan/take a picture and upload it in the file upload section of this form.

Sample 2

If your ticket is blue, complete the back of the ticket. Scan/take a picture of the front and back and upload the two files in the file upload section of this form.
